Job Summary

We are seeking a skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will work closely with our clients and their families to develop personalized care plans, ensuring their unique needs are met. Your role will involve:

  • Conducting thorough assessments to identify client needs and preferences
  • Developing and implementing care plans in collaboration with clients, families, and healthcare professionals
  • Providing ongoing support and guidance to clients and their families
  • Collaborating with our care team to ensure seamless delivery of care services

Requirements

To be successful in this role, you will need:

  • Experience in a care coordination or complex care setting
