We are seeking a highly skilled and dedicated Community Nurse to join our team in Gravesend, North Kent. As a Community Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our Community Nursing team.
Key Responsibilities- Provide high-quality patient care and support to individuals and families in the community.
- Work collaboratively with other healthcare professionals to deliver comprehensive care and support.
- Develop and implement individualized care plans to meet the unique needs of patients.
- Conduct regular assessments and evaluations to ensure the best possible outcomes for patients.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Registered Nurse with current NMC registration.
- Experience working in community nursing or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to work a variety of shifts, including evenings and weekends.
