About the Role
We are seeking a dedicated and compassionate Band 5 Community Staff Nurse to join our team in Gravesend, North Kent. As a Community Nurse, you will have the opportunity to work closely with patients and their families, providing high-quality care and support in a community setting.
Key Responsibilities
- Deliver high-quality patient care, working independently and as part of a multidisciplinary team.
- Develop and implement individualized care plans, taking into account the unique needs and goals of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Provide education and support to patients and their families, promoting health and wellness in the community.
