Care Coordinator for Complex Needs
About the Role:We are seeking a dedicated and compassionate nurse to join our specialist rehabilitation unit in Norfolk. The unit provides comprehensive care and support to adult females with complex needs, including autism, personality disorders, and mental health issues.
Key Responsibilities:- Deliver high-quality nursing care to patients with diverse and complex needs.
- Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and the multidisciplinary team.
- Administer medications and treatments in accordance with professional standards and regulatory requirements.
To be successful in this role, you will need:
- Registered Nurse with valid NMC Pin.
- Experience working in a learning disability or mental health setting is highly desirable.
