Children and Families Mediator and Advocate

- Develop and implement tailored support plans that meet the unique needs of each family.
- Provide guidance and support to families, empowering them to make informed decisions about their lives.
- Collaborate with external agencies to ensure seamless referrals and support.
- Monitor progress and evaluate the effectiveness of support interventions.
- NVQ level 3 or an equivalent qualification in social care or a related field.
- English and Maths or an equivalent qualification at level 2 of the National Qualification Framework.
- A minimum of 2 years' experience working with children, young people, and families in a health or social care setting.
