Adult Safeguarding Care Coordinator

An exciting opportunity has arisen for a dedicated individual to join the South Warwickshire University NHS Foundation Trust as an Adult Safeguarding Assistant Practitioner.
This role is based in Nuneaton, but you will have the chance to work across North Warwickshire, providing care and support to patients in their own homes under minimal supervision.
Key Responsibilities- Deliver high-quality patient care, assessing, implementing, and evaluating treatment plans with the guidance of registered professionals.
- Work collaboratively with the multidisciplinary team to ensure effective patient care, maintaining confidentiality at all times.
- Communicate effectively with patients, relatives, and colleagues, using various mediums to meet individual needs.
- Supervise and support junior staff, promoting a positive working environment.
As a successful candidate, you will possess excellent communication skills, be able to work autonomously, and demonstrate a caring and compassionate nature.
