The Adult Care Coordinator for Adults and Families role is an exciting opportunity to join our team at WMJobs. This challenging position involves providing a high-quality social work service to older adults, families, and carers in a pioneering hospital acute setting.
Key Responsibilities- Providing a social work service to a highly professional standard
- Undertaking and recording robust assessments and care and support planning
- Working closely with the Multidisciplinary Health & Social Care team
- A Professional Social Work qualification and registration with Social Work England
- A clean driver's license
- Adult social care experience and good IT skills

We estimate the salary for this role to be around £42,000 - £52,000 per annum, depending on location and experience.
