We are seeking a skilled and compassionate Adult Safeguarding Assistant Practitioner to join our team in South Warwickshire.
The successful candidate will possess a Foundation Degree in Health and Social Care or equivalent, along with significant experience in delivering care in a healthcare environment.
About the Job Description
- This is a Band 4 position within our Urgent Response/Community Response Team, working under minimal supervision to deliver high-quality patient care.
- The successful candidate will be responsible for assessing, implementing, and evaluating patient care needs, ensuring the delivery of excellent care while adhering to Trust policies and procedures.
- Able to work autonomously, prioritize tasks effectively, and maintain accurate records.
- Excellent communication skills, both written and verbal, are crucial for this role, as well as a willingness to participate in training and development activities.
