Community Mental Health Nurse

An exciting opportunity has arisen for a newly qualified or graduate Band 5 Nurse to join the East London NHS Foundation Trust's Newham Home Treatment and Crisis Assessment Team. This dynamic community-based service provides urgent triage responses and comprehensive crisis assessments to adult service users living in the London Borough of Newham.
Main Duties- Conduct in-depth crisis assessments and formulate treatment and risk management plans with the support of a multi-disciplinary team.
- Provide short-term, intensive, safe, and effective home-based treatment as an alternative to acute adult in-patient care.
- Work collaboratively with colleagues to deliver high-quality care and support to service users.
- Registered Nurse Part 3 and 13 (NMC) or Nurse trainee/graduate.
- Experience of making full psycho-social assessments and writing reports.
- Experience of working as part of a multi-professional team.
- Ability to communicate effectively both verbally and in writing.
- Leadership skills and ability to manage self and others.
