Job Summary

We are seeking a skilled Care Manager to join our team. As a Care Manager, you will play a critical role in supporting children and families in need, providing guidance and resources to help them thrive.

Key Responsibilities
  • Develop and implement individualized care plans to meet the unique needs of each child and family
  • Collaborate with a network of professionals to ensure comprehensive support and services
  • Provide education and support to families on mental health topics and resources
  • Conduct home visits and community outreach to engage with families and provide support
  • Document progress and outcomes in a timely and accurate manner
Requirements
  • Bachelor's degree in social work, psychology, education, or a related field
  • Two years of experience working with youth facing mental health challenges
  • Excellent communication and interpersonal skills
  • Ability to use own vehicle for local travel
