Job Title: Leaving Care Specialist
Description:
We are seeking a skilled and compassionate Leaving Care Specialist to join our team at Service Care Solutions Ltd. As a Leaving Care Specialist, you will be responsible for providing high-quality support to young people transitioning from care into independent adulthood.
- Establish and maintain strong relationships with young people, providing ongoing support and guidance throughout their journey.
- Collaborate with partner agencies to ensure seamless support and address any crisis situations that may arise.
- Facilitate access to the Local Offer for care leavers, including the Living Well Hub and its drop-in service.
- Support care leavers living independently in the community, equipping them with the necessary resources and encouragement.
- Requirements:
- Minimum NVQ Level 3 or equivalent qualification relevant to the role.
- At least 5 GCSEs, including Maths and English.
- Understanding of the Children Leaving Care Act 2000 and related responsibilities.
