Job Summary

We are seeking a qualified Nurse to join our team as a Nurse Care Specialist. As a Nurse Care Specialist, you will provide person-centered care to our clients, working closely with our care management team to ensure the best possible outcomes.

Key Responsibilities

  • Provide high-quality care to clients in their own homes, following their individual care plans and in line with our Health and Safety policy.
  • Communicate effectively with clients and their families, building strong relationships and ensuring their needs are met.
  • Work closely with our care management team to ensure seamless care delivery and continuous improvement.
  • Keep accurate records of client interactions and care provided, using our care management app.

Requirements

  • Qualified Nurse (Adult preferable) Band 5 and above.
  • Valid NMC Pin.
  • Previous experience working in domiciliary care (desirable but not a requirement).
