Barts Health NHS Trust is committed to providing high-quality care to our patients, and we are seeking a highly skilled and experienced Neonatal Community Outreach Nurse to join our team.
Main Responsibilities- Provide a seamless and high-quality service from referral to discharge, enabling timely discharge for babies less than 36 weeks gestation.
- Offer help and support to families during their transition to home by preparing them with training and teaching, such as Basic Life Support (BLS), Nasogastric Tube (NGT) feeds, and encouraging self-confidence.
- Be a point of reference for staff and parents, offering specialist community-based advice.
- Liaise with multidisciplinary colleagues as needed, including Speech and Language Therapists (SLT) and Dieticians.
- Participate in ward rounds and weekly Multidisciplinary Team (MDT) meetings.
- Work towards autonomous practice, in conjunction with other team members and the leading consultant.
- Monitor the quality and standard of care provided by the outreach team by completing monthly audits.

Person SpecificationThe ideal candidate will have:
- NMC registration.
- Professional registration.
- Teaching and mentoring qualification.
- Specialist and intensive care experience.
Desirable criteria include:
- BLS qualification.
- NLS qualification.
The successful candidate will have significant post-registration experience, experience at Band 6, and evidence of dissemination of knowledge to the wider healthcare team, as well as evidence of parent teaching and data collection database. They will also have sound clinical reasoning and decision-making skills.
The ideal candidate will have knowledge of the implementation of community management plans.
