District Nurse/Senior Community Health Practitioner

We are seeking an experienced and skilled District Nurse to join our Integrated Teams in Barking and Dagenham. As a key member of our community nursing team, you will be responsible for providing high-quality nursing care to patients in their homes and community settings.
About the RoleThis is an exciting opportunity for a motivated and compassionate nurse to lead and manage a community nursing team, working closely with GPs, other healthcare professionals, and statutory and voluntary agencies to deliver individualized care to patients.
Key Responsibilities- Lead and manage a community nursing team, providing clinical leadership and guidance to ensure high-quality patient care.
- Assess, plan, implement, and evaluate patient care, working in partnership with patients, families, and other healthcare professionals.
- Develop and maintain effective relationships with GPs, other healthcare professionals, and statutory and voluntary agencies to ensure seamless care coordination.
- Contribute to the development and implementation of community nursing policies, procedures, and guidelines.
- Participate in quality improvement initiatives and contribute to the development of a culture of excellence within the team.
To be successful in this role, you will need:
- A District Nurse qualification and current NMC registration as a Registered Nurse (RN).
- Experience of team working, interagency collaboration, and leadership.
- Excellent communication, interpersonal, and leadership skills.
- A good understanding of NHS priorities, clinical governance, and the NMC Code of Professional Conduct.
