Job Title: District Nurse/Senior Community Nurse

We are seeking an experienced and skilled District Nurse to join our Integrated Teams in Barking and Dagenham. As a key member of our team, you will be responsible for providing high-quality nursing care to patients in the community, working closely with GPs, other professionals, and statutory and voluntary agencies.

Main Responsibilities:
  • Lead and manage a Community Nursing Team, geographically aligned to GP practices
  • Responsible for the management of a caseload, ensuring the provision of a high standard of nursing care to patients and their families in the community
  • Work as part of the integrated primary/community health care team, working within and across professional and organisational boundaries
  • Liaise with GPs, other professionals, and statutory and voluntary agencies to deliver individualised care to patients
Requirements:
  • District nurse qualification, certificate, DiP, or Degree
  • Current NMC Registration Registered Nurse (RN)
  • Nurse/Prescriber/Non-Medical Prescriber
  • Evidence of professional up-dating
