Community Health Nurse

We are seeking a highly skilled and dedicated Registered Nurse to join our Unplanned Care team at NELFT North East London Foundation Trust. As a Community Health Nurse, you will play a vital role in providing high-quality care to patients in the community.
Key Responsibilities- Assess, manage, plan, and deliver care to patients allocated to you, ensuring professional and legal accountability.
- Triage and provide care to patients in the borough of Barking and Dagenham, adhering to the CHSCS SOP.
- Work collaboratively with colleagues to ensure 7-day coverage between 7am and 7pm, demonstrating flexibility and a commitment to team work.
- Current NMC/RN or equivalent 1st-level Nursing qualification.
- Evidence of post-registration education and training.
- Desirable qualifications include Mentor qualification, relevant post-registration courses, and Nurse Prescriber (V150).
- Awareness of NHS plan, NSF, and clinical governance priorities.
- Ability to initiate patient assessment and care planning with senior colleague support.
- Experience working with professionals from other disciplines and providing packages of care involving other agencies.
