About the Role

We are seeking a dedicated and innovative Registered Nurse to join our Unplanned Care team in Barking and Dagenham. As a Community Health Specialist, you will be responsible for providing high-quality care to patients in their homes, working closely with other healthcare professionals to ensure seamless care delivery.

Main Responsibilities

The successful candidate will be responsible for:

  • Assessing and managing patient care plans, including triaging and providing urgent visits as needed
  • Working collaboratively with the community health and social care team to ensure comprehensive care delivery
  • Providing education and support to patients and their families to promote independence and self-care
  • Participating in quality improvement initiatives to enhance patient outcomes and experience

Requirements

To be successful in this role, you will need:

  • A current NMC registration and a relevant nursing qualification
  • Experience of working in a community health setting, with a focus on unplanned care
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
  • A commitment to delivering high-quality patient care and contributing to quality improvement initiatives
