About the Role

We are seeking a highly skilled and dedicated Registered Nurse to join our Unplanned Care team in Barking and Dagenham. As a Community Health Specialist, you will be responsible for providing high-quality care to patients in their own homes, working closely with other healthcare professionals to ensure seamless care delivery.

Key Responsibilities

  • Assess, manage, plan, and deliver care to patients allocated to you
  • Triage and provide care to patients in the borough of Barking and Dagenham requiring ad hoc/urgent visits
  • Work collaboratively with the unplanned care team to ensure 7-day coverage between 7am and 7pm

Requirements

  • Current NMC registration as a Registered Nurse
  • Experience of working in a community nursing setting
  • Ability to work autonomously and as part of a team
