Job Title: Clinical Dietitian - Community Specialist

About the Role:
We are seeking a highly skilled and motivated Clinical Dietitian to join our Community Specialist team in Havering. As a Clinical Dietitian, you will be responsible for providing high-quality dietetic care to patients across the community, working closely with multidisciplinary teams to improve nutritional outcomes.
Key Responsibilities:
- Conduct dietetic assessments and provide evidence-based treatment plans for patients with complex nutritional needs.
- Work collaboratively with healthcare professionals to develop and implement care plans that promote optimal nutrition and health.
- Provide education and support to patients, carers, and healthcare professionals on nutrition and diet-related topics.
- Contribute to the development and implementation of nutrition-related policies and procedures.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
- Registered with the Health and Care Professions Council (HCPC) as a Dietitian.
- Member of the British Dietetic Association (BDA).
- Minimum of 2 years' experience working in community dietetics.
- Complex enteral feeding and nutrition support experience.
- Experience working with multidisciplinary teams and providing education to healthcare professionals.
