About the Role

We are seeking a skilled and dedicated Community Nurse to join our Unplanned Care team in Barking and Dagenham. As a Community Nurse, you will play a vital role in providing high-quality care to patients in their own homes, working closely with other healthcare professionals to ensure seamless care delivery.

Key Responsibilities
  • Assess, manage, plan, and deliver care to patients allocated to you, ensuring professional and legal accountability.
  • Triage and provide care to patients within the borough of B & D requiring ad hoc/urgent visits, in line with the CHSCS SOP.
  • Work collaboratively with the unplanned care team to ensure 7-day coverage between 7am and 7pm, demonstrating flexibility and a commitment to teamwork.

Requirements
  • Current NMC/RN or equivalent 1st-level Nursing qualification.
  • Evidence of post-registration education and training.
  • Desirable: Mentor qualification, relevant post-registration courses, and Nurse Prescriber (V150) certification.
