About the Role

We are seeking a highly skilled and dedicated Registered Nurse to join our Unplanned Care team at NELFT North East London Foundation Trust. As a Community Health Nurse, you will play a vital role in providing high-quality care to patients in the community.

Key Responsibilities
  • Assess, manage, plan, and deliver care to patients allocated to you, ensuring professional and legal accountability.
  • Triage and provide care to patients in the borough of Barking and Dagenham requiring ad-hoc/urgent visits, in line with the CHSCS SOP.
  • Work collaboratively with colleagues to ensure cover 7 days a week between 7am and 7pm, demonstrating flexibility and a commitment to team work.
Requirements
  • Current NMC/RN or equivalent 1st-level Nursing qualification.
  • Evidence of post-registration education and training.
  • Ability to initiate patient assessment and care planning with the support of a senior colleague.
  • Experience of working with professionals from other disciplines and providing packages of care involving other agencies.
