Job Title: District Nurse/Senior Community Nurse

We are seeking an experienced and skilled District Nurse/Senior Community Nurse to join our Integrated Teams in Barking and Dagenham. As a key member of our team, you will be responsible for providing high-quality nursing care to patients in the community, working closely with other healthcare professionals to deliver individualized care plans.

About the Role

This is an exciting opportunity for a motivated and compassionate nurse to lead and manage a Community Nursing Team, working geographically aligned to GP practices. You will be responsible for the management of a caseload, ensuring the provision of a high standard of nursing care to patients and their families in the community.

Key Responsibilities
  • Lead and manage a Community Nursing Team, providing leadership and guidance to team members
  • Develop and implement individualized care plans, working closely with patients, families, and other healthcare professionals
  • Assess, manage, and deliver care to patients in the community, including those with complex needs
  • Collaborate with other healthcare professionals, including GPs, social workers, and therapists, to deliver comprehensive care
  • Supervise pre-registration students and provide mentorship and support to junior staff
Requirements

To be successful in this role, you will need:

  • Current NMC Registration as a Registered Nurse (RN)
  • District nurse qualification, certificate, or DiP
  • Experience of team working, interagency collaboration, and leadership
  • Excellent interpersonal, communication, and leadership skills
