Job Summary

An exciting opportunity has arisen for a skilled and dedicated Mental Health Practitioner to join our Assertive Outreach Service. As a key member of our team, you will play a vital role in providing comprehensive care packages for individuals with severe and enduring mental health problems.

About the Role

As a Mental Health Practitioner, you will be responsible for assessing, planning, delivering, and evaluating care for service users who are referred to our Assertive Outreach Service. This will involve working within a psychosocial framework to assess and manage risk, and developing therapeutic relationships with service users to provide support and assistance.

You will also be responsible for administering and monitoring the effects of prescribed medication, and liaising with medical staff as necessary. Additionally, you will possess enhanced communication skills to effectively communicate with service users and carers who may be experiencing psychotic symptoms and be in distress.

Key Responsibilities
  • Assess, plan, deliver, and evaluate care for service users referred to the Assertive Outreach Service
  • Develop therapeutic relationships with service users to provide support and assistance
  • Administer and monitor the effects of prescribed medication
  • Liaise with medical staff as necessary
  • Communicate effectively with service users and carers who may be experiencing psychotic symptoms and be in distress
Requirements

We are looking for a skilled and dedicated Mental Health Practitioner who possesses the following qualifications and experience:

  • RMN or Registered Mental Health Nurse qualification
  • Excellent assessment, risk management, organisational, and communication skills
  • Experience and good working knowledge of the Care Programme Approach, Community mental health legislation, and Hospital admission/discharge process
