About the Role

We are seeking a highly motivated and experienced Mental Health Practitioner to join our Assertive Outreach Service team. As a key member of our team, you will be responsible for assessing, planning, delivering, and evaluating care for service users who are referred to our service.

Key Responsibilities
  • Assess and manage risk, working within a psychosocial framework to develop individual care plans.
  • Organize and plan your work as an autonomous practitioner, liaising closely with other agencies and disciplines.
  • Undertake the role of Care Coordinator/Lead Professional for a caseload of service users with complex mental health problems.
  • Develop therapeutic relationships with service users to provide support and assistance.
  • Administer and monitor the effects of prescribed medication, liaising with medical staff as necessary.
  • Offer advice and information to clients and carers regarding prescribed medications.
  • Communicate effectively with service users and carers who may be experiencing psychotic symptoms or be in distress.
  • Undertake the role of Lead Clinician for individuals detained under a Community Treatment Order.
