Job Summary

An exciting opportunity has arisen for a dedicated and compassionate Mental Health Practitioner to join our Assertive Outreach Service. As a key member of our team, you will play a vital role in providing comprehensive care packages for individuals with severe and enduring mental health problems.

About the Role

As a Mental Health Practitioner, you will be responsible for assessing, planning, delivering, and evaluating care for service users who are referred to our Assertive Outreach Service. You will work closely with other agencies and disciplines to provide a holistic approach to care, focusing on the psychosocial and family-centered needs of our service users.

You will possess excellent assessment, risk management, organizational, and communication skills, with a good working knowledge of the Care Programme Approach, Community Mental Health legislation, and Hospital admission/discharge processes. Your ability to use your initiative, manage your time effectively, and work as part of a team will be essential in this role.

Key Responsibilities
  • Assess and manage risk for service users with complex mental health problems
  • Develop and implement comprehensive care packages, working within a psychosocial and family-centered framework
  • Provide support and assistance to service users, developing therapeutic relationships to promote engagement and empowerment
  • Administer and monitor the effects of prescribed medication, liaising with medical staff as necessary
  • Offer advice and information to clients and carers regarding prescribed medications
  • Lead Clinician for individuals detained under a Community Treatment Order, completing formal tribunal/hospital managers reports as required
