About the Role

We are seeking a highly skilled and dedicated Acute Care Nurse to join our Crisis Resolution and Home Treatment Team at Lincolnshire Partnership NHS Foundation Trust.

As a Band 5 Clinician, you will play a vital role in providing high-quality care to individuals experiencing acute mental illness, working closely with a multidisciplinary team to assess, treat, and support service users in their recovery journey.

Key Responsibilities
  1. Assessment and Treatment: Conduct comprehensive assessments of service users, identifying their mental health needs and developing personalized care plans to support their recovery.
  2. Crisis Intervention: Provide timely and effective interventions to individuals in crisis, working closely with the Crisis Team to ensure safe and effective care.
  3. Collaboration and Communication: Develop and maintain effective relationships with service users, carers, and other healthcare professionals to ensure seamless care coordination and delivery.
  4. Risk Management: Identify and mitigate risks associated with service user care, ensuring the safety and well-being of individuals and staff.
  5. Documentation and Record-Keeping: Maintain accurate and timely clinical records, adhering to Trust policies and procedures.
Person Specification

We are looking for a highly motivated and experienced Registered Nurse with a strong background in mental health care. The ideal candidate will possess:

  1. Registration with the NMC: Hold current registration with the Nursing and Midwifery Council (NMC).
  2. Specialized Training: Evidence of specialized continued professional training in clinical practice, with a focus on mental health care.
  3. Knowledge and Skills: Sound knowledge of clinical/risk assessment and understanding of Information Governance principles, with the ability to apply this knowledge in a fast-paced and dynamic environment.
  4. Experience and Qualifications: Relevant experience of working with mental health needs, with a strong understanding of the national agenda for mental health.
