The successful candidate will have excellent clinical skills, leadership, and management qualities, with the ability to implement high standards of patient care and ensure a positive patient experience. You will work closely with our community team leader to develop and implement innovative solutions to challenging situations, ensuring that patients receive timely and effective care.
Main Duties of the Job
The District Nurse Caseload Manager will be responsible for the day-to-day smooth running of the caseload, holding clinical responsibility for the patients they are managing and leading on. This will involve ensuring that patients are appropriately assessed and care is delivered in a timely manner, with the involvement of patients and carers as appropriate.
You will also be responsible for ensuring that team members are appropriately trained, supported, and have the right knowledge, skills, and competencies to meet the requirements of the role.

Person Specification
The successful candidate will have current NMC registration or a relevant allied healthcare professional qualification. They will also have experience of care co-ordination/care management and an awareness of current Health and Social Care policies.
