Wellbeing Mentor

We are seeking a highly motivated and experienced Wellbeing Mentor to join our team at Bevan Community Benefit Society. As a Wellbeing Mentor, you will play a vital role in delivering a coordinated and high-quality social prescribing service to support clients in accessing and engaging with community resources.
Key Responsibilities- Develop trusting relationships with service users, empowering them to take control of their health and wellbeing.
- Conduct client needs assessments, using a Wellbeing questionnaire to assess the impact of interventions.
- Work collaboratively with the practice team to identify and manage a caseload of clients, referring them to community support services as needed.
- Provide ongoing support and guidance to clients, promoting independence and self-management.
- GCSE grade A-C or equivalent in English & Maths, with a degree in a relevant field (e.g., health, social care, or education).
- Demonstrable experience of working with people with multiple health and social needs, with a sound knowledge of local support agencies.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
