Wellbeing Coach

We are seeking a highly motivated and experienced Wellbeing Mentor to join our team at Bevan Community Benefit Society. As a Wellbeing Mentor, you will play a vital role in delivering a coordinated and high-quality social prescribing service to support clients in accessing and engaging with community resources.
Key Responsibilities- Develop trusting relationships with service users, empowering them to take control of their health and wellbeing by identifying their strengths and assets.
- Conduct client needs assessments in the practice, community, or via home visits, using a Wellbeing questionnaire to assess the impact of interventions.
- Work collaboratively with the practice team to support a caseload of clients, proactively identifying those who would benefit from social prescribing and managing referrals to community support services.
- Refer clients to the voluntary sector or local community support services after an agreed period of case management, making recommendations on non-clinical approaches that can complement existing interventions.
We are looking for an individual with:
- GCSE grade A-C or equivalent in English & Maths, and educated to A-level standard or equivalent.
- Demonstrable experience of working with people with multiple health and social needs, with a sound knowledge of local support agencies and a commitment to ongoing professional development.
- Excellent communication skills, both verbally and in writing, with the ability to work collaboratively with colleagues and partner organisations.
