Wellbeing Coach

We are seeking a highly motivated and experienced Wellbeing Mentor to join our team at Bevan Community Benefit Society. As a Wellbeing Mentor, you will play a vital role in supporting individuals to take control of their health and wellbeing, and connect them with community groups and statutory services for practical and emotional support.
Key Responsibilities- Develop trusting relationships with service users, giving them time to focus on their assets and what matters to them, and providing them with personalized help to take control of their health and wellbeing.
- Undertake client needs assessments in the practice, community, or via occasional home visits, using a Wellbeing questionnaire pre- and post-interventions to assess the impact on the client's wellbeing.
- Work as part of the practice team to support a caseload of clients for whom social prescribing might offer improved outcomes, and manage and regularly review your caseload to accommodate urgent referrals for support as required.
- Following an agreed period of case management, refer clients on to the voluntary sector or local community support services where appropriate, and make recommendations on where a non-clinical approach might support the patient better or complement existing clinical interventions and improve outcomes.
We are looking for an individual with:
- GCSE grade A-C or equivalent in English & Maths, and educated to A-level standard or equivalent.
- Demonstrable experience of working with people with multiple health and social needs within a healthcare setting, or community-based work with socially excluded groups.
- Excellent communication skills, both verbally and in writing, and IT literacy with a sound working knowledge of Microsoft packages and data management/case recording systems.
