About the Role

Bevan Community Benefit Society is seeking a skilled Wellbeing Mentor to join our team. As a Wellbeing Mentor, you will work collaboratively with our healthcare colleagues to deliver a coordinated and high-quality social prescribing service. Your primary goal will be to support clients in accessing and engaging with community resources, empowering them to take control of their health and wellbeing.

Key Responsibilities

  • Develop trusting relationships with service users, focusing on their strengths and assets, and providing personalized support to promote independence and self-management.
  • Conduct client needs assessments in the practice, community, or via home visits, using a Wellbeing questionnaire to evaluate the impact of interventions.
  • Work as part of the practice team to support a caseload of clients, proactively identifying those who would benefit from social prescribing and managing referrals to community support services.
  • Refer clients to the voluntary sector or local community support services after an agreed period of case management, making recommendations on non-clinical approaches that can complement existing interventions.

Requirements

  • GCSE grade A-C or equivalent in English & Maths, with a degree in a related field or equivalent experience.
  • Demonstrable experience working with people with multiple health and social needs, with a sound knowledge of local support agencies and a commitment to ongoing professional development.
  • Excellent communication skills, IT literacy, and a working knowledge of Microsoft packages and data management systems.
