About the Role

Bevan Community Benefit Society is seeking a highly skilled and compassionate Wellbeing Mentor to join our team. As a Wellbeing Mentor, you will play a vital role in delivering a coordinated and high-quality social prescribing service to support clients in accessing and engaging with community resources.

Key Responsibilities

  • Develop trusting relationships with service users, empowering them to take control of their health and wellbeing by identifying their strengths and assets.
  • Conduct client needs assessments in the practice, community, or via home visits, using a Wellbeing questionnaire to assess the impact of interventions.
  • Work collaboratively with the practice team to support a caseload of clients, proactively identifying those who would benefit from social prescribing and managing referrals to community support services.
  • Refer clients to the voluntary sector or local community support services after an agreed period of case management, making recommendations on non-clinical approaches that can complement existing clinical interventions.

Person Specification

  • GCSE grade A-C or equivalent in English & Maths, with a degree in a related field or equivalent experience.
  • Demonstrable experience of working with people with multiple health and social needs, with a sound knowledge of local support agencies and a commitment to ongoing professional development.
  • Excellent communication skills, both verbally and in writing, with the ability to work effectively with diverse groups and individuals.
