We are seeking a highly motivated and experienced Wellbeing Mentor to join our team at Bevan Community Benefit Society. As a Wellbeing Mentor, you will play a vital role in delivering a coordinated and high-quality social prescribing service to support clients in accessing and engaging with community resources.
Key Responsibilities- Develop trusting relationships with service users, empowering them to take control of their health and wellbeing through personalized support and guidance.
- Conduct client needs assessments, using a Wellbeing questionnaire to assess the impact of interventions and inform case management.
- Work collaboratively with the practice team to identify and manage a caseload of clients, referring out to community support services as needed.
- Follow up with clients after an agreed period of case management, making recommendations on where a non-clinical approach might support improved outcomes.
We are looking for an individual with:
- GCSE grade A-C or equivalent in English & Maths, and educated to A-level standard or equivalent.
- Demonstrable experience of working with people with multiple health and social needs, with a sound knowledge of local support agencies.
- Excellent communication skills, both verbally and in writing, with a commitment to equality of opportunity and anti-discriminatory practices.
As a Wellbeing Mentor at Bevan Community Benefit Society, you will have the opportunity to work in a dynamic and supportive environment, contributing to the development of our social prescribing service and making a positive impact on the lives of our clients.
