We are seeking a qualified Senior Community Mental Health Practitioner to join our experienced team at Southern Health NHS Foundation Trust in Southampton Central Community Mental Health Team.

As a Senior Mental Health Practitioner, you will provide a care coordination role as a member of a multi-disciplinary team, engaging, supporting and working with a caseload of service users with severe or enduring mental health problems.

You will help service users manage their symptoms, deliver evidence-based interventions and give advice and support on issues such as housing, benefits, education, employment, medication and social activities in the local area.

The successful candidate will be able to assess and monitor service users' mental state, provide comprehensive risk assessment and implement a management plan, undertake crisis management and contingency planning.

You will work in collaboration with service users as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user.

Southern Health is one of the largest NHS Foundation Trusts in the UK, specialising in mental health and learning disabilities, as well as offering physical health community-based services.

We are committed to providing the best possible healthcare, reaching a range of diverse communities, whilst placing patients and staff at the forefront of all our endeavours.

The salary for this role is estimated to be around £40,000 - £50,000 per annum, depending on experience.
