About the Opportunity

As a Day Support Worker, you will be responsible for providing person-centered care and support to individuals with complex needs.
You will work collaboratively with our multidisciplinary team to develop and implement individualized care plans that promote independence, community integration, and overall well-being. Key Responsibilities
- Provide physical and emotional support to individuals with complex needs
- Assist with daily living tasks and activities of daily living
- Participate in multidisciplinary team meetings to share knowledge and expertise
