About the Role

As a highly skilled Advanced Nurse Practitioner, you will be responsible for providing comprehensive care to patients in Bramhall and Cheadle Hulme Primary Care Network. This role involves assessing, diagnosing, and treating patients from initial history taking to evaluation of care.

You will work closely with the primary care team to provide seamless pathways of care, making referrals as necessary. With your expertise, you will contribute to the practice's quality targets, helping to sustain high standards of patient care and service delivery.

Key Responsibilities:
  • Assess and triage patients, including same-day triage, and provide definitive treatment or make necessary referrals.
  • Manage undifferentiated conditions, identify red flags, and take appropriate action.
  • Use complex decision-making to inform diagnosis, investigation, and complete management of episodes of care.
  • Provide personalized care and population-centred approaches to enable shared decision-making with patients.
  • Complete relevant training to provide multi-professional clinical practice and CPD supervision to other roles.
Requirements:
  • Masters qualification for Advanced Clinical Practice or similar accreditation.
  • NMC registration.
  • Evidence of Continued Professional Development.
  • Experience working in Primary Care.
  • Ability to prioritize and organize own case load.
  • Excellent communication and interpersonal skills.
