Children's Health Care Professional

This role offers a unique chance to specialise in paediatrics and work closely with the paediatric multidisciplinary team (MDT) and other specialist teams, including paediatric outpatient and home enteral feeding dietetic teams.
Key Responsibilities:- Develop and implement comprehensive dietetic plans for children and their families/carers, encompassing assessment, advice, treatment, and counselling.
- Independently manage a complex caseload, working collaboratively with children and their carers to agree patient-centred care goals.
- Contribute to the development of coordinated care plans by working closely with the wider MDT and attending relevant meetings and case conferences.
- Regularly evaluate patients' progress and participate in service improvement initiatives to enhance patient outcomes.
- Promote effective communication among patients, carers, and healthcare professionals to ensure seamless care delivery.
- Train and educate patients, carers, colleagues, and other healthcare professionals to foster a unified approach to patient care.
- Maintain awareness of clinical risks within your caseload and adhere to safeguarding principles.
