About the Role:

The successful applicant will work as part of the St Andrew's Healthcare team, supporting service users and their carers through various aspects of their treatment stay.

Key Responsibilities:

  1. Manage a workload agreed with the Lead Social Worker and MDT, prioritizing tasks to ensure effective service delivery and meet performance expectations, while also ensuring compliance with relevant policies and procedures.
  2. Implement The Mental Health & Mental Capacity Act in daily work, ensuring best practices and guidelines are followed to deliver high-quality services.
  3. Prepare and present reports for Mental Health Review Tribunals and Managers Hearings, showcasing expertise and presenting clear, concise information to inform decision-making.
  4. Develop and present Social History reports and reports for Clinical Reviews, highlighting key findings and recommendations to improve service delivery and outcomes.
  5. Attend relevant multi-disciplinary meetings, collaborating with community social workers, other agencies, and professionals to achieve shared goals and objectives, and promoting interagency cooperation.
  6. Engage in 1-1 and group work with service users, maintaining accurate records in clinical notes of all interactions and interventions, and evaluating effectiveness.
  7. Liaise with service users' families/carers and other statutory/voluntary agencies, building strong relationships and partnerships to enhance service delivery and user satisfaction.
  8. Undertake child visit and home visit risk assessments, recommending actions to the MDT to ensure safety and well-being, and minimizing risks.
  9. Lead on planning for service users' discharge and aftercare arrangements, considering safeguarding and multi-agency public protection procedures to ensure seamless transitions and optimal outcomes.
  10. Identify, visit, and assess resources suitable for service users moving into the community or residential settings, promoting independence and integration into society, and reducing reliance on services.
  11. Adhere to MAPPA processes, making referrals and attending MAPPA meetings as required, to protect vulnerable individuals and communities, and uphold professional standards and responsibilities.
  12. Conduct S42 enquiries as directed by the Local Authority, ensuring compliance with relevant legislation and upholding professional standards and responsibilities.
  13. Lead on day-to-day coordination of safeguarding, ensuring proactive measures are in place to prevent harm and promote a culture of safety, and respond to emerging threats and risks.
  14. Participate actively in Social Work team meetings, contributing ideas and expertise to drive positive change and continuous improvement, and enhancing service delivery and user satisfaction.
  15. Regularly attend prearranged supervisions with the Lead/Senior Social Worker, preparing agenda items to address key issues and develop personal and professional skills, and receive feedback and coaching.
  16. Demonstrate awareness of mental health and childcare legislation, applying this knowledge throughout work at St Andrew's Healthcare to deliver high-quality services and promote user-centered care.
  17. Pursue mandatory training and attend advised training sessions to maintain professional registration with SWE, staying updated on industry developments and best practices, and enhancing career prospects.
  18. Deliver Social Work-based training within the Division/Hospital as needed, sharing expertise and enhancing colleague capabilities and confidence, and promoting a culture of learning and development.
  19. Complete the Social Work post-qualifying ASYE, further developing skills and competencies, and enhancing career prospects and job security.

Salary:£48,000 - £58,000 per annum, based on experience and qualifications, and reflecting the importance of this role in delivering high-quality services.
