About the Role

We are seeking a dedicated Care Coordinator to join our Complex Care Division in Sussex. As a Care Coordinator, you will be responsible for managing client caseloads and collaborating with clients, their families, carers, and the wider coordination team to cover shift requirements.

The successful candidate will have previous care coordination experience and excellent interpersonal skills. They will be able to work autonomously and as part of a wider team, with a genuine interest in working within a caring environment.

Key Responsibilities:

  • Manage client caseloads and collaborate with clients, their families, carers, and the wider coordination team
  • Proactively address day-to-day events and maintain high standards in communication and event management
  • Play a key role in implementing and maintaining Superior Healthcare's quality assurance programme

Requirements:

  • Previous care coordination experience
  • Excellent interpersonal skills and a 'team player'
  • Ability to work autonomously and as part of a wider team
  • Observant and sensitive to a carer's and client's needs
  • Excellent oral and written communication skills
  • Good IT skills including use of Microsoft Teams & Outlook
