About the Role

We are seeking a dedicated Care Coordinator to join our Complex Care Division in Sussex, Falmer. As a key member of our team, you will be responsible for managing client caseloads and collaborating with clients, their families, carers, and the wider coordination team to ensure high-quality, safe, and compassionate care.

The ideal candidate will have previous care coordination experience, excellent interpersonal skills, and a genuine interest in working within a caring environment. You will be an ambassador for the company, working within our values and maintaining excellent working relationships with clients.

Key responsibilities include:

  • Managing client caseloads and coordinating care plans
  • Collaborating with clients, their families, carers, and the wider coordination team
  • Implementing and maintaining our quality assurance programme
  • Proactively addressing day-to-day events and maintaining high standards in communication and event management
