Job Summary:
The successful candidate will work closely with our residents, building strong relationships and empowering them to achieve their desired goals and outcomes. You will be responsible for coordinating and delivering a range of frontline care and support activities, including risk assessments, safeguarding checks, and welfare visits. Your expertise will also be essential in developing and implementing personalized support plans, ensuring that each individual receives tailored care and attention.
Main Responsibilities:
- Coordinate and deliver frontline care and support activities, including risk assessments, safeguarding checks, and welfare visits.
- Develop and implement personalized support plans, working closely with residents and multidisciplinary teams to ensure tailored care and attention.
- Build strong relationships with residents, fostering trust and confidence in your ability to provide expert support.
- Work collaboratively with colleagues to share best practices, enhance service delivery, and improve outcomes for residents.
- Maintain accurate records and reports, adhering to data protection policies and procedures.
Requirements:
To succeed in this role, you will need:
- A genuine passion for supporting vulnerable individuals and making a positive impact on their lives.
- Proven experience in a similar role, preferably within a social care or health setting.
- Excellent communication and interpersonal skills, with the ability to build rapport with people from diverse backgrounds.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
- A commitment to maintaining confidentiality and adhering to data protection policies and procedures.
