About the Job

This is an exciting opportunity to join our team as a Care Coordinator for Learning Disabilities at Dane House in Brighton. You will be responsible for coordinating the care and support of individuals with complex needs, working closely with our multidisciplinary team to ensure that each individual receives the highest quality care and support.

As a Care Coordinator, you will need to possess excellent communication and organisational skills, as well as the ability to work effectively with a wide range of stakeholders, including families, carers, and other healthcare professionals.

In this role, you will also have the opportunity to contribute to the development of our care plans and strategies, ensuring that we continue to deliver high-quality care and support to our residents.
