Care Coordinator for Frailty Same Day Emergency Care

As a highly skilled and dedicated nurse, you will play a pivotal role in delivering exceptional care to our elderly patients in the Frailty Same Day Emergency Care Unit at University Hospitals Sussex NHS Foundation Trust.
About the RoleWe are seeking an enthusiastic and motivated Registered Nurse to join our team as a Care Coordinator for Frailty Same Day Emergency Care. The ideal candidate will have substantial experience in frailty care and excellent communication skills, both written and verbal.
Key Responsibilities:
- Deliver high-quality patient care in the designated specialist area, influencing practice within the multidisciplinary team
- Conduct comprehensive assessments of patients using theoretical knowledge and extensive experience to identify needs and develop effective care plans
- Coordinate interventions with other healthcare professionals through case management, multidisciplinary team meetings, and goal setting
- Ensure accurate and up-to-date documentation, maintaining confidentiality and adhering to data protection policies
- Develop and implement sessions of care based on sound theoretical knowledge and evidence-based practices
- Utilize clinical governance frameworks and resources to guide practice, engaging in supervision and reflection on clinical decision-making
