Children's Health Outreach Nurse

We are seeking a highly motivated Registered Nurse to join our Children's Services team at East Suffolk and North Essex NHS Foundation Trust. As a Children's Health Outreach Nurse, you will play a vital role in providing high-quality care to children and their families in our community.
Key Responsibilities- Assess the needs of children and their families, and develop and implement individualized care plans.
- Provide expert nursing care to children with a wide range of health needs, including those with life-limiting conditions and complex healthcare requirements.
- Work collaboratively with other healthcare professionals to ensure seamless care transitions and effective communication with families.
- Maintain accurate and up-to-date clinical records, and contribute to the development of policies and procedures to improve patient care.
- Participate in education and training programs to enhance your skills and knowledge, and contribute to the development of the Children's Services team.

Person Specification- Proven experience of children's nursing, with a focus on outreach and community care.
- Evidence of managing and improving the patient experience, with a focus on family-centered care.
- Experience of working with children with life-limiting conditions and complex healthcare needs.
- Evidence of safe guarding experience, with a focus on protecting children and families.
- Mentoring of students, with a focus on developing the next generation of healthcare professionals.
