About the Role

We are seeking a highly motivated and experienced Registered Nurse to join our team as a Children and Young People Health Outreach Nurse. The successful candidate will have a proven track record of working with children and young people in a healthcare setting and will be passionate about delivering high-quality care and support.

The Children and Young People Health Outreach Nurse will work closely with our multidisciplinary team to provide care and support to children and young people in their own homes and in the community. The role will involve assessing the needs of children and young people, developing and implementing care plans, and providing education and support to families and carers.

The ideal candidate will have excellent communication and interpersonal skills, be able to work autonomously, and have a strong commitment to delivering high-quality care and support. They will also have a good understanding of the needs of children and young people and their families, and be able to work effectively with other agencies and professionals.

Key Responsibilities
  • To assess the needs of children and young people and develop and implement care plans
  • To provide education and support to families and carers
  • To work closely with the multidisciplinary team to provide care and support to children and young people
  • To maintain accurate and up-to-date records of care and support provided
  • To contribute to the development of policies and procedures related to children and young people's care and support
Requirements
  • Registered Children's Nurse
  • Experience of working with children and young people in a healthcare setting
  • Excellent communication and interpersonal skills
  • Ability to work autonomously
  • Strong commitment to delivering high-quality care and support
