Children and Young People Health Outreach Nurse

We are seeking a highly motivated Registered Nurse to join our Children's Services team. As a key member of our team, you will be responsible for providing high-quality nursing care to children and their families.
Key Responsibilities- Assess the needs of children and their families, and develop and implement individualized care plans.
- Provide evidence-based nursing care, including medication administration, wound care, and vital sign monitoring.
- Liaise with multidisciplinary teams to ensure seamless care transitions and effective communication.
- Maintain accurate and up-to-date clinical records, and communicate concerns to the nurse in charge as needed.
- Participate in quality improvement initiatives to enhance patient care and safety.
