Children's Health Outreach Nurse

We are seeking a highly motivated Registered Nurse to join our Children's Services team at East Suffolk and North Essex NHS Foundation Trust. As a Children's Health Outreach Nurse, you will play a vital role in providing high-quality care to children and their families in our community.
Key Responsibilities- Assess the needs of children and their families, and develop and implement individualized care plans.
- Provide evidence-based nursing care to children with a wide range of health needs, including those with life-limiting conditions and complex healthcare requirements.
- Work collaboratively with the multi-disciplinary team to ensure seamless care transitions and effective communication with families.
- Maintain accurate and up-to-date clinical records, and communicate concerns to the nurse in charge as necessary.
- Participate in mentoring and education programs to support the development of nursing staff and students.

Person Specification- Proven experience of children's nursing, with a focus on community-based care.
- Evidence of managing and improving the patient experience, with a focus on family-centered care.
- Experience of working with children with life-limiting conditions and complex healthcare needs.
- Evidence of safe guarding experience, with a focus on protecting children and their families.
- Mentoring of students, with a focus on developing their clinical skills and knowledge.
