Children's Health Outreach Nurse

We are seeking a highly motivated Registered Nurse to join our team, who has proven experience working within an acute hospital setting and wishes to develop their clinical and leadership skills.
The Children's Services team covers a wide range of areas, including the Children's Assessment Unit, Children's Emergency Department, Children's Elective Care, and Children's Inpatient Unit. Our busy departments receive children with a variety of illnesses and injuries, offering exceptional opportunities for learning, development, and career progression.
Main Responsibilities:- Provide nursing procedures and patient care, with minimal supervision, within the scope of professional practice.
- Assess needs, plan, implement, and evaluate evidence-based nursing care.
- Carry out prescribed care and treatment, as necessary, within Trust/Unit policy parameters.
- Maintain accurate clinical records, communicating concerns to the nurse in charge promptly.
- Liaise with nursing and medical colleagues, and other disciplines, to ensure continuity of patient care during hospitalization and discharge.
- Ensure child and family privacy and confidentiality at all times.
- Maintain good communication with patients, relatives, multi-disciplinary teams, and other visitors to the ward/unit.

Person Specification:- Proven experience of children's nursing.
- Experience of working with children with life-limiting conditions/complex healthcare needs.
- Evidence of safeguarding experience.
- Registered Children's Nurse.
