Learning Disability Liaison Nurse

As a Learning Disability Liaison Nurse, you will play a vital role in ensuring that individuals with learning disabilities receive equal access to healthcare services. You will work closely with clinicians and services to develop action plans that improve service delivery and health outcomes for people with learning disabilities.
Key Responsibilities:
- Support primary and secondary care to exercise their duty to provide people with learning disabilities with equal access to healthcare services.
- Develop action plans to improve service delivery and health outcomes for people with learning disabilities.
- Work with identified individuals within primary and secondary care to achieve the goals set out in the action plan.
- Promote services in line with current local and national policy and guidance for people with learning disabilities.
- Undertake all aspects of practice as an autonomous practitioner and report to the Band 6 accordingly.
Requirements:
- RN-LD or equivalent RN qualification.
- Professional, clinical knowledge acquired through training to degree/diploma level.
- Experience of working with people who have a learning disability.
- Ability to assess and manage risk effectively.
- Enhanced DBS Check.
