About the Role

We are seeking a highly skilled and dedicated Learning Disability Liaison Nurse to join our team at Devon Partnership NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in ensuring that individuals with learning disabilities receive high-quality care and support within our acute hospital setting.

The successful candidate will have a strong background in learning disability nursing and a passion for delivering person-centered care. You will work closely with our multidisciplinary team to develop and implement care plans that meet the unique needs of each individual, promoting their autonomy, dignity, and well-being.

Key responsibilities will include:

  1. Providing expert nursing care and support to individuals with learning disabilities, ensuring their physical and emotional needs are met.
  2. Working collaboratively with our multidisciplinary team to develop and implement care plans that promote independence, dignity, and well-being.
  3. Facilitating open and easy access to hospital services for individuals with learning disabilities, ensuring their needs are met and their rights are respected.
  4. Providing education and training to healthcare professionals and other stakeholders on learning disability care and support.
  5. Contributing to the development and implementation of policies and procedures that promote person-centered care and support for individuals with learning disabilities.
