Learning Disability Liaison Nurse

We are seeking a highly skilled and dedicated Learning Disability Liaison Nurse to join our team at Devon Partnership NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in ensuring that individuals with learning disabilities receive high-quality care and support within our acute hospital setting.
The successful candidate will have a strong background in learning disability nursing and a passion for delivering person-centered care. You will work closely with our multidisciplinary team to develop and implement care plans that meet the unique needs of each individual, promoting their autonomy, dignity, and well-being.
Key responsibilities will include:
- Providing expert nursing care and support to individuals with learning disabilities, ensuring their physical and emotional needs are met.
- Working collaboratively with our multidisciplinary team to develop and implement care plans that promote independence, dignity, and well-being.
- Facilitating open and easy access to hospital services for individuals with learning disabilities, ensuring their needs are met and their rights are respected.
- Providing education and training to healthcare professionals and other stakeholders on learning disability care and support.
- Contributing to the development and implementation of policies and procedures that promote person-centered care and support for individuals with learning disabilities.
